3. Disciplinary Actions

 

Step Three: Final Counseling Session

If performance does not improve following the Written Warning, or if the employee is again in violation of Erwine Home Health and Hospice practices, rules or standards of conduct, a final counseling session may occur. The final counseling session is the third and final step of Erwine’s disciplinary process.

During the final counseling session, the manager will again discuss the problem with the employee, emphasizing the seriousness of the issue and the need to immediately remedy the problem. The manager will advise the employee that he or she is now at the third and final formal level of disciplinary action and will advise him or her that, if a positive change in behavior does not occur, or if another disciplinary problem occurs, he or she will be terminated. Employees unwilling to make such a commitment may either resign or be terminated. After the meeting, the manager will write a letter summarizing the employee’s commitment and the consequences of failing to meet this commitment. The employee will be required to sign the letter to acknowledge receipt. A copy of the letter and receipt will be placed in the employee’s personnel file.

Erwine Home Health and Hospice reserves the right not to follow the disciplinary procedure and move directly to termination. That decision will be based on the situation and in the judgment of the Human Resources Department that this is the best course of action for the organization.
