All full time employees will be eligible the first of the month following 90 days of full-time service with no days off.

Employee pays 100% of employee, spouse and dependent coverage cost. Refer to program benefit booklet for plan details.

PDO-Vacation,

All full time employees accrue PDO time immediately. Minimum of 78 hours (or 52 patient visits in a pay period in the case of Per Visit Registered Nurse/Therapist Full Time Employees or 60 patient visits in a pay period in the case of Per Visit Licensed Practical Nurse Full Time Employees) must be worked in order to calculate accrued time

Full time employees are eligible to use accrued time after completion of 6 months of full time employment with no days off. No time will accrue if employee works less than 78 hours in a pay period (or performs less than 52 patient visits in a pay period in the case of Per Visit Registered Nurse/Therapist Full Time Employees or 60 patient visits in a pay period in the case of Per Visit Licensed Practical Nurse Full Time Employees). The maximum hours which can be used as a basis for PDO accrual for one pay period 80 hours and, with respect to Per Visit Full Time Employees, the maximum patient visits which can be used as a basis for PDO accrual for one pay period is 54 patient visits for Per Visit Registered Nurse/Therapist Full Time Employees or 62 patient visits in a pay period in the case of Per Visit Licensed Practical Nurse Full Time Employees.

PDO – Status Change

Employee changing from full time to part time after one year of employment.

Employee will retain PDO time for use while employed at Erwine Home Health and Hospice.
